

If you're struggling with deep, relentless fatigue, you've likely had your ferritin levels checked. Perhaps you were told your levels are "fine," or maybe you've been choking down oral iron supplements for months with little to no progress.
When you're bottomed out, an iron infusion can feel like a daunting step. You might be wondering what the procedure actually feels like, how quickly your energy will return, and if there are side effects you should worry about.
As a naturopathic doctor practicing since 2007, I've spent years helping patients navigate iron deficiency.
For the first half of my career, oral iron was our primary tool for treating low ferritin stores, particularly in women. We just had to cross our fingers and hope patients could tolerate the digestive side effects and actually absorb it. The old alternative—iron injections into the muscle—carried a permanent risk of tattooing the skin. So, oral iron it was.
I'll admit my perspective changed entirely when I found myself on the patient side of the equation. And thankfully treatments have advanced since back then!

Not long ago, I discovered I was not only severely iron-deficient with low ferritin, but I was also fully anemic; my red blood cells were physically smaller than normal. I immediately started taking oral iron. After six long months of strict supplementation, my ferritin only crawled from 14 ug/L to 36 ug/L.

Knowing that 36 ug/L was still far below the optimal level of 100 ug/L, I decided to learn how to administer intravenous iron infusions to heal myself. After just four treatments, my ferritin jumped from 36 to 104 ug/L. I've successfully maintained that optimal level ever since.
I know firsthand exactly what it feels like to be completely depleted, and I know what it feels like to finally be fully resourced. Now I'm on a mission to help other people. I keep saying to patients, "Join me up here! The view is amazing!"
The most common, undeniable change people notice after an iron infusion is a return of energy. While some feel it almost immediately and massively, for others, it builds steadily over the course of their treatment series.
A positive shift is noticeable in the mornings. Patients find they can wake up more quickly, shake off the morning grogginess, and spend their days with renewed physical, emotional, and mental stamina.
Beyond general energy, there are several unexpected benefits that consistently surprise my patients:
Tackling Deferred Projects: I love hearing patients tell me they finally had the mental and physical bandwidth to conquer a daunting task they had been putting off for months—like finally organizing the entire garage.
The Period Paradox: Many women are thrilled to find that their menstrual periods become both lighter and shorter once their iron stores are adequate. It sounds like a paradox, but when your body has enough iron, you actually tend to lose less blood during your monthly cycle.
Cosmetic and Physical Relief: Patients routinely celebrate when their hair finally stops shedding, their brittle nails strengthen, and chronic, low-grade headaches disappear.
In British Columbia, the Lifelabs algorithm marks anything above 100 ug/L as a normal iron store. However, the official cutoff for being "definitely iron deficient" is often set as low as 15 ug/L.
This creates a confusing "no man's land" between 15 and 100 ug/L where thousands of patients get lost in the medical system. Medical doctors look at a patient with a ferritin level of 20 ug/L and say, "You're fine! There's nothing we can do." Meanwhile, that patient is completely exhausted and struggling to function.
To make matters more complicated, ferritin is an acute-phase reactant, meaning it rises with inflammation. If a patient has systemic inflammation—indicated by a High-Sensitivity C-Reactive Protein (Hs-CRP) value above 5—their ferritin reading will be falsely elevated.
In these complex cases, we have to look deeper at iron and transferrin saturation. If your transferrin saturation is below 20%, you're clinically iron deficient, even if your ferritin looks normal on paper.
In my practice, I use an easy rule of thumb for inflamed patients: take the baseline ferritin value and divide it by three to find the iron stores clinically available to the body.
When you couple these diagnostic hurdles with the fact that oral iron absorption can be lower than 5%—while causing miserable side effects like constipation—IV iron infusions become an incredibly logical, efficient, no-brainer choice.
When it comes to the actual appointment, the type of iron I use in my clinic is Iron Sucrose, which is exceptionally well tolerated. It's a very neutral feeling IV, it shouldn't burn, sting, or be painful. It can feel cool (since you're warmer than the beg), heavy, or like mild pressure.
Every body is different, but here is what you can realistically expect during and right after your appointment:
A Surprising Taste: When I give myself an iron infusion, I distinctly notice a mapley bacon taste. A handful of my patients have reported the exact same thing! It is a quirky, harmless side effect that always brings a bit of humor to the room.
Rare Mild Reactions: While headaches, a general feeling of malaise, or a mild, non-allergic skin rash are technically possible, they are incredibly uncommon with Iron Sucrose, and I've yet to see them manifest in my patients.
The "Healing Rest" Fatigue: A small percentage of patients feel a bit tired right after their appointment. Because iron-deficient patients are already exhausted, this is rarely a disruptive change. My clinical theory is that your body is finally receiving a vital nutrient it has been starved of for a long time. To utilize that iron and truly heal, your body needs to rest. Usually, energy bounces back the very next day, often higher than your baseline before the infusion.

Because Iron Sucrose requires multiple doses, we customize your treatment plan using a precise and handy spreadsheet to calculate your exact iron deficit.
We know that every time we administer 100 mg of iron during a 45-minute office visit, we can expect a predictable 12.5-point increase in ferritin. I typically start a patient with 50 mg or 100 mg to see how they respond, and then titrate up to 200 mg per IV session depending on their physical constitution, goals, and budget.
Most patients require a series of 4 to 5 treatments to hit that optimal 100 ug/L mark. For example, starting with a baseline ferritin of 12 ug/L means we need an 88-point increase. Administering one 100 mg dose followed by three 200 mg treatments will land that patient perfectly at an optimal 99.5 ug/L.
To get the absolute best results from your iron infusions, timing and consistency are key.
Condense the Schedule: I prefer the initial treatments to be scheduled as closely together as possible, ideally once a week for 4 to 5 weeks. Treatments can also be twice per week.
Time it with Your Cycle: My absolute favorite strategy for menstruating patients is to start their infusion series right as their period begins. This allows us to get all or most of the treatments completed before the next cycle hits, letting the patient effectively get ahead of the monthly blood loss.
Post-Treatment Testing: Once the initial series is complete, we wait about four weeks before re-testing your blood to see where your levels have landed and determine if a final top-up or two are needed.
Long-Term Maintenance: If you're a menstruating woman, your body is continuously losing iron. To keep your energy high and avoid bottoming out again, most women benefit immensely from a single treatment or a short series every 2 to 3 months.
An iron infusion isn't just about changing a number on a lab report; it is about giving your body the fundamental resources it needs to wake up easily, think clearly, and live vibrantly.
